FYI: Tooth Whitening 

With summer approaching, many are asking about tooth whitening for a brighter smile. Our office has provided whitening services since 1992, with good results. Several approaches are popular today. Each has its benefits and drawbacks. Nearly all the bleaching products use peroxide to bleach the enamel of teeth. This discussion does not include whitening toothpastes, which often depend upon abrasives for surface scrubbing. Before whitening, we strongly recommend an examination to ensure teeth and oral structures are healthy, and to insure that whitening is appropriate.

 

1. Over-the-counter products, such as Crest Whitestrips and Oral B products have been heavily marketed to the consumer and are popular. Their advantage is simplicity. A disadvantage is that the strip or delivery tray is not customized to fit. The result can be irregular whitening or difficulty keeping the product on the teeth. Many of our patients who have tried them have had satisfactory results, however.

 

2. The most widely recommended product in our office is office-assisted home whitening. We make a custom tray from an impression. Our staff provides instruction in the use of the product, provides solutions to common problems such as sensitivity, and adjusts the type of whitener so that the best result with minimal discomfort is achieved. Patients typically come into the office every week or two during active whitening so our staff can monitor the results and address any problems that may develop. Active treatment time varies widely, but is typically no longer than 4-6 weeks. We recommend whitening only during the treatment time. Further use of whitening products after the brightness has reached its peak is not recommended. We do encourage a one or two day touchup with whitener every 6-12 months that conveniently coincides with periodic examinations and cleanings.

 

3. In-office whitening fits the needs of those who find the use of trays at home to be inconvenient. Dr. Valo has used ZOOM whitening for many years. ZOOM is a light activated peroxide that is applied after isolating the teeth with paint-on gels and retractors. Sometimes, there is more initial sensitivity with this. We find that the faster teeth are whitened, the greater the sensitivity, but this has not been a problem that lasts beyond a day or two. To counter sensitivity, a special desensitizing toothpaste is used in advance of application, and a desensitizing gel is provided at the completion of the whitening session. Again, ZOOM whitening has best served those who wish to avoid the use of whitening trays at home. Many, however, use ZOOM whitening as a “jump start” on whitening and they use trays at home to enhance the whitening result.

 What’s new? We are investigating in-office whitening that does not use light activation. Our hope is that better results with greater comfort can be obtained in the category of in-office whitening.

FYI: 2011 AACD Meeting in Boston...

During the week of May 15-22 Dr. Valo was in Boston for the annual meeting  of the American Academy of Cosmetic Dentistry.  World-class teachers led workshops and lectures aimed at improving our dental skills.  Topics ranged from application of new filling materials, understanding biology related to dental implants, creating crowns and fillings that undetectable in a smile, to smile design.   Dr. Valo was involved in one of several workshops focused on creating lifelike texture in fillings and veneers of front teeth.  We are looking forward to applying this knowledge to enhance your dental health and appearance!
